When you go for a tattoo?

This question is for the tattoo artist. Is it awkward for you guys to do nipple piercings and to do genitalia piercings on people? Do you guys have to give yourselves a pep talk or do you just go in and do the deed? I've always wondered about this and I have to applaud those that go down under and do this.
